INDICATIONS FOR USE OF POTASSIUM CITRATE

Indications for use of potassium citrate are:

  • calcium nephrolithiasis (in patients with low urinary citrate excretion);
  • renotubular acidosis and metabolic acidosis (in chronic renal failure), without hyperkalemia.

Additionally, potassium citrate is also used to supplement potassium deficiency in the body and in diseases that require chronic urine alkalization.

POTASSIUM CITRATE DOSAGE

The preparation is administered orally. The dose is determined individually for each patient. The dose depends on the pH of urine and the amount of citrate excreted in urine per day. The usual dose is 50 milligrams per kilogram of body weight.

CONTRAINDICATIONS TO THE USE OF POTASSIUM CITRATE

Contraindications to the use of potassium citrate include hyperkalemia (high blood potassium levels), urinary tract infections, circulatory failure, blood clotting disorders, alkalosis (especially metabolic), vascular diathesis, severe heart damage, ammonium-phosphate-magnesium stone disease, chronic renal failure, inflammation of the stomach and duodenum, gastrointestinal bleeding, delayed gastrointestinal transit, planned surgical procedures associated with the risk of blood loss.

SPECIAL WARNINGS AND PRECAUTIONS FOR THE USE OF POTASSIUM CITRATE

During treatment with oral potassium citrate, it is important not to chew the tablets as this may increase the irritating effect on the gastric mucosa.

During treatment, it is important to control urine acidity (recommended urine pH 6.5-7) or measure the daily urinary citrate excretion. The most important thing in the treatment of kidney stones is to take the drug before going to bed and immediately after waking up in the morning.

The drug should be taken long-term to prevent the development of the disease, which is why systematic use and laboratory monitoring of urine and blood as well as appropriate dietary recommendations are so important, especially in people with kidney disorders.

Particular caution should be exercised when administering potassium citrate concomitantly with potassium-sparing diuretics, ephedrine and pseudoephedrine, opioids, drugs affecting blood clotting, macrolide antibiotics, and urine alkalizing or acidifying drugs.

CONTRAINDICATIONS OF POTASSIUM CITRATE FOR COMBINATION WITH OTHER ACTIVE SUBSTANCES

Potassium citrate should not be administered concurrently with magnesium and aluminum hydroxide, methenamine and eplerenone.

POTASSIUM CITRATE INTERACTIONS WITH FOOD

You should limit the use of foods rich in potassium (tomatoes, oranges, fruit compotes). It is recommended to administer the drug after a meal to limit possible irritation of the gastrointestinal tract.

POTASSIUM CITRATE INTERACTIONS WITH ALCOHOL

You should not drink alcoholic beverages during treatment with potassium citrate.

EFFECT OF POTASSIUM CITRATE ON DRIVING

Potassium citrate does not affect the ability to drive or operate machinery.

EFFECT OF POTASSIUM CITRATE ON PREGNANCY

There are no adequate clinical studies on pregnant women. Taking the drug during this period should be under the supervision of a gynecologist and nephrologist.

EFFECT OF POTASSIUM CITRATE ON LACTATION

No studies on safety of use in breastfeeding mothers.

SIDE EFFECTS

hyperkalemia, nausea, vomiting, respiratory disorders, dizziness, heartburn, spots before the eyes, hemorrhagic diathesis, peripheral vascular disorders, irritation of mucous membranes including the gastric mucosa.

SYMPTOMS OF POTASSIUM CITRATE OVERDOSE

An overdose of potassium citrate may lead to hyperkalemia. Nausea and vomiting, pallor and hypotension, muscle cramps and numbness of the extremities, and cardiac arrhythmia may occur. Metabolic alkalosis (increased blood pH above 7) may occur in patients with renal failure.

MECHANISM OF ACTION OF POTASSIUM CITRATE

Citrates are a group of chemical compounds that are inhibitors of the reaction of kidney stone formation, especially calcium stones. Citrates slow down the formation of calcium oxalate and calcium phosphate crystals, and also have urine alkalizing properties. This causes a decrease in urine saturation with calcium salts, which are stone-forming.

POTASSIUM CITRATE ABSORPTION

Citrates are absorbed in the intestines very quickly and efficiently, with very little dependence on food content. Potassium citrate is absorbed in 90% of the administered dose.

POTASSIUM CITRATE METABOLISM

Potassium citrate is metabolized in the intestinal wall to bicarbonate, which is an alkalizing agent. A small portion of citrate enters the bloodstream.

POTASSIUM CITRATE EXCRETION

Potassium citrate is excreted by the kidneys.
